Getting Started

Like most things in life, the first step is often the hardest. We have created this website to help make the process easy for you. At Wadle and Associates, our comprehensive team of mental health professionals gives you the options you need to find the provider that is the best fit for your needs, your schedule, and your insurance situation.

Step #1: Insurance
Take a few minutes to review your insurance coverage.
On the back of your insurance card is an 800 number. Call the number to learn about the mental health benefits available to your specific policy.
What is the extent of my mental health coverage?
Do I need to get pre-authorization for mental health treatment?
What is the co-pay due at the time of each appointment?
Does my policy cover the Wadle provider I have selected?

Step #2: Choose a Provider
On the Our Providers page, you will find information about each member of our provider team. You can learn about the focus areas they serve, their experience and the insurance companies they work with. Browse through the list and note a few providers that are a good fit for your situation.
If you prefer, you can call our office at 515-270-1344, and our support staff can help you choose a provider.

Step #3: Make an Appointment
Call our office at 515-270-1344. Our support staff will help you schedule your first appointment and confirm your insurance information.

Step #4: Patient Forms
The paperwork required takes about 15 minutes to complete. On the day of your first appointment, please arrive early enough time to complete the forms.
Wadle and Associates offers online patient forms as well. You are welcome to download and complete the required patient forms prior to your first appointment. The adult packet is for patients 18 and over. The child packet is for patients 17 and younger.

Step #5: Your First Appointment
Please arrive 10 minutes before your appointment time.
Don’t forget to bring:
Your insurance card
Completed forms

Check in with the front desk. Our support staff will make a photocopy of your insurance card and review your information. If you have completed forms prior to the appointment, please give them to our support staff at the front desk at this time.
